AESOP Pain Management

Consulting face to face with our pain specialist and then remotely for follow up.

To set this up:
Step # 1 must have been completed:

  1. Fill out online forms: New patient sign-up form, pre-consultation questionnaire, medical/social history form and informed consent form with an original signature.
  2. Submit your medical records stating your diagnosis, lab work (CMP-14 and CBC no more than 12 months old), sent in a copy of your driver's license and have sent in the results of your drug urinalysis.
  3. After all items have been received and accepted, and you are ready to set up your consultation.

Step # 2

  1. Go to www.yahoo.com and download yahoo messenger.
  2. Purchase a webcam if you do not already have one.
  3. Send an e-mail to skittlesaesop@yahoo.com in order to let us know that you have completed the above steps and are ready to test your webcam via yahoo messenger. Include your yahoo messenger screen name so we can add you to our contact list. We will send this information to the doctor as well.
  4. You and the administrative assistant, will test your webcam; we can then set up your consultation with the doctor.
  5. We will set a time, date and contact number if you do not have messenger with voice.
  6. An appointment will be set by our administrative assistant in our Indianapolis office.
  7. The doctor will discuss your condition with you. You and the doctor will decide the best regimen for your situation.
  8. Your request/prescription will be sent to the pharmacy. The pharmacy will contact you once you prescription has been received via the telephone number provided.
  9. You will need to send an e-mail or instant message to the administrative assistant @ skittlesaesop@yahoo.com or skittlesaesop on messenger, to confirm your shipping address and desired shipping method.
